Getting Cleared for Athletic Participation
Before students participate in tryouts, practice, or competition, they must be cleared by the Athletic Directors.

To be cleared, students must complete the following:
  1. Completed and signed EMERGENCY CARD*
  2. Completed and signed ATHLETIC PERMIT CARD*
  3. Completed PHYSICAL EXAMINATION CARD* signed by a licensed physician, physicians assistant, or licensed nurse practitioner
  4. Proof of health insurance (photocopy of insurance card)
  5. Signed ATHLETIC HANDBOOK pp. 27, 28, 29, and 30 (available under "Quick Links")
  6. $25 athletic fee ($30 if received after the season start date) payable to the Riverside Bookstore or in the Main Office (Rm. 213)
*Available in the Athletic Office (Rm. 156)

Students who falsify information (including forging signatures) on athletic paperwork will be suspended from all athletic practice and participation for 365 days.
